
Patient Diagnosis
| ICD-10 Code | The diagnosis code of the patient. |
| List Order | The primary order of diagnosis in customer chart.TIP: The first qualifying diagnosis will normally be pulled into new orders. |
Infectious Disease
| Airborne | Indicates patient has an infectious disease that is airborne contagious. |
| Contact | Indicates patient has an infectious disease that is contact contagious. |
| Droplet | Indicates patient has an infectious disease that is blood contagious. |

| Employment | If the patient’s condition is related to his/her employment, then you will need to check this box. |
| Auto Accident | If the patient’s condition is related to an automobile accident, then you will need to check this box. |
| Other Accident | If the patient’s condition is related to any other accident, then you will need to check this box. |
| State | The state where the patient had an auto accident if their condition is related to an auto accident. |
| Date | The date the accident occurred. |
| (Do Not Resuscitate) | Indicates patient has chosen not to be resuscitated in the event they are incapacitated. |
