| Item ID | Displays the Item ID and description of the Item. |
| UM (Package) | The package or unit attached to a Coverage Criteria Tie number. |
| Tie No | The Coverage Criteria to which this Item/Package is attached. To view/select a Coverage Criteria, click the ellipse button to display a list. |
| HCPCS Procedure | Displays the procedure code of the Coverage Criteria. |
| Payor | Displays the Payor Name of the Coverage Criteria. |
| Units | The number of units which the Item/Package represents. |
