Physician, Ordering	The physician who ordered this customer these items.
Referring	The physician who referred this customer these items.
Referral	The person or company who referred this order.
Rendering	The physician who renders this customer these items.
Salesperson	The salesperson responsible for this order.
Tax Table	The sales tax code to assign to this order.
Status	Select a status for this medical order.
Customer Service	The Customer Service person is set initially to the user who entered the Order initially.
Location	The Location from which Order will be delivered.
